Nortriptyline is a tricyclic antidepressant in use since the 1950s to treat depression. More recently it has seen widespread off-label use, with patients buying nortriptyline to treat chronic pain, labile affect, migraines, and other conditions. It is also used in very low doses to help prevent childhood bedwetting in patients at least 5 years of age.

Uses of Nortriptyline

This medication has a wide range of uses in addition to its traditional role as an antidepressant; as an antidepressant it has fallen somewhat out of favor as newer, more effective medications have been developed. It is, however, still highly effective in treating melancholic depression, depression which does not originate from grief and is hallmarked by the inability to find any pleasure in positive events.

Patients often buy nortriptyline to treat:

• Melancholic depression
• Depression when other medications don't work
• Childhood bedwetting
• Several types of chronic pain
• Temporomandibular joint dysfunction (TMD, TMJD)
• Panic attacks
• Labile affect
• Migraine prevention
• Irritable bowel syndrome

For depression, regardless of the type, treatment typically starts with lower doses for a couple of weeks and is titrated up to 75-100 mg per day, taken in multiple smaller doses or one large dose. Results will be noticeable after a couple of weeks of treatment, with full effects felt within one month:

• Take 25 mg three or four times per day; or
• Take 100 mg once per day.
• Do not exceed 150 mg per day.

For bedwetting, in children ages 5 and up, physical causes should first be ruled out. The medication's antispasmodic effect on the bladder is likely what helps reduce accidents. Doses are very low and treatment lasts no more than three months:

• Take 10 mg once per day shortly before bed.
• Do not exceed 15 mg per day.

If buying nortriptyline for other uses patients would do best to speak with a doctor about dosage and duration of treatment, which may range from days to years. Treatment can also be as-needed, common for some types of panic attacks.

Note that for migraines, nortriptyline has been shown to reduce the frequency and severity of migraine headaches, but will not address pain when a migraine does happen. Other medications, such as triptans, should be utilized during a migraine episode.

If nortriptyline has been used for more than a few weeks at moderate doses, treatment should not end abruptly. Doing so can cause withdrawal symptoms of anxiety, insomnia, malaise, or motor disturbances. Treatment is best slowly reduced over time; a doctor can help with a discontinuation plan.
